Answer a few questions about a request for a binder, certificate, or evidence of insurance. Get the form, the reasoning, and the traps to avoid. Scoped to proof-of-coverage requests only — new business applications, no-loss letters, and supplements aren't covered yet.

This tool is a guide for issuing ACORD proof of insurance. It is not state-specific, and reflects guidelines in effect as of 09-2026, using ACORD forms only. Verify that the writing company actually uses ACORD forms and that your agency has authority to issue them before relying on any result here. Built using ISO policy language and ACORD form guidelines.

Practice notes — every form, every line

Only what's in the policy. Never write anything onto a binder, certificate, or evidence form — including cancellation notice language, "100% replacement cost," or "guaranteed replacement cost" — that isn't actually granted by the policy or a carrier-issued endorsement. If it's not approved in the policy, it doesn't go on the form.
AI / WOS boxes. Only check the Additional Insured or Waiver of Subrogation boxes on a certificate when the holder is individually scheduled on the policy or endorsement. If their status comes only from a blanket AI/WOS endorsement, leave those boxes unchecked.
Replacement Cost Estimator requests. If a lender asks for the RCE itself, or asks to confirm the property is insured to 100% of replacement cost, don't disclose the estimator. Respond that the building is insured to 100% of replacement cost as calculated by the carrier — the RCE is the carrier's proprietary information.
Listing endorsements. When a certificate or evidence form calls for endorsements to be listed, list only the exact form name, number, and edition date. Don't paraphrase or add wording that isn't specifically printed on the policy without underwriter approval.
Multi-line certificates and client privacy. A single ACORD 25 can show GL, Auto, Umbrella/Excess, and Workers' Comp together. If the request doesn't say which lines the client wants shown, confirm with the client before issuing — don't disclose coverage lines they didn't ask to have shared.
